Kindred Transitional Care and Rehabilitation - Oak Hill is a 139-bed skilled nursing and rehabilitation center located in Pawtucket, Rhode Island, which is considered part of the greater Providence metropolitan area. Located in the Oak Hill section of Pawtucket and adjacent to Providence’s fashionable East Side, it is uniquely situated approximately two miles from Brown University and the Warren Alpert School of Medicine and two major teaching hospitals, The Miriam Hospital and Memorial Hospital of Pawtucket. Five additional teaching hospitals are located within five miles. Access to the facility is convenient from Interstate 95, Exit 27, from points North and South.

We are improving our clinical capabilities and staffing in order to accept patients with greater medical complexity than a traditional nursing home. This is increasingly important as many higher acuity patients require transitional care before they are ready to return home.
