Five-Star Rating, November, 2012

Centers for Medicare and Medicaid Services (CMS) created the Five-Star Quality Rating System to help consumers, their families, and caregivers compare nursing homes more easily and to help identify areas about which you may want to ask questions. The quality rating system gives each nursing home a rating of between 1 and 5 stars. Nursing homes with 4 or 5 stars are considered to have “above average” quality.

America’s Best Nursing Homes, 2011 and 2012

U.S. News and World Reports annually recognizes nursing centers that have received the highest-rankings from the Centers for Medicare and Medicaid Services. Nursing centers are rated on measures of care, nurse staffing and health inspections.

Silver – Achievement in Quality Award, 2009

Our efforts received national recognition when this center was named the recipient of an American Health Care Association (AHCA)/National Center for Assisted Living (NCAL) Silver – Achievement in Quality Award.

The AHCA/NCAL Quality Award is modeled after criteria from the Baldrige Performance Excellence Program, the nation’s premier recognition for quality achievement.

The Silver award recognizes that the center is dedicated to performance measures, sustainable organizational and process results, and continuous quality and performance improvement.

Deficiency-Free Survey, 2011

Kindred Transitional Care and Rehabilitation Center - Oak Hill received a deficiency-free survey from the Rhode Island Department of Health in 2011.

Our center received a deficiency-free survey from the Department of Health. State regulations require each facility to be thoroughly surveyed and evaluated on areas such as safety, quality of care, patient rights, food service, nursing care and administration - we excelled in meeting all of these requirements. Each state conducts these onsite inspections that determine whether its nursing homes meet the minimum Medicare and Medicaid quality and performance standards. The State conducts inspections of each nursing home that participates in Medicare and/or Medicaid about once a year. The inspection team consists of trained inspectors, including at least one registered nurse.

Best in Class – Response to Problems Award, 2009 and 2010

Pinnacle Quality Insight is an independent customer satisfaction firm serving more than 1,100 health care providers in 45 states. The Best in Class awards are given to nursing centers scoring in the top 10% for specific areas of quality service. Centers are ranked based upon monthly patient, resident, and family survey results, as well as side-by-side comparisons with similar nursing centers across the nation.

Distinguished Administrator Award, 2009
Rhode Island Health Care Association (RIHCA) honored our Executive Director, Scott Sanborn, with the Distinguished Administrator award in 2009.

RIHCA established the Distinguished Service awards in 2001 to recognize volunteers and professionals working in Rhode Island’s skilled nursing and rehabilitation centers. Candidates are nominated by RIHCA members and are awarded based upon the individual’s dedication to providing high quality healthcare to those living in nursing centers.

Executive Director of the Year, 2004
Kindred Healthcare’s Nursing Center Division honored our Executive Director, Scott Sanborn, with the Executive Director of the Year award in 2004.

Each year, Kindred recognizes an Executive Director as a recipient of the Executive Director of the Year award. The winner is chosen by the Executive Vice President and President of the Nursing Center Division based on the individual’s excellent leadership in achieving high resident and employee satisfaction, quality outcomes, and other performance measures.

Our center is accredited by The Joint Commission. The Joint Commission is a private, not for profit organization established in 1951 to evaluate healthcare organizations that voluntarily seek accreditation.

A Fortune Magazine Most Admired Company, 2009 – 2013
Kindred Healthcare, our center’s parent company, has been a Fortune magazine Most Admired Healthcare Company for the last five years.  The Most Admired list is the definitive report card on corporate reputations.  Fortune magazine ranks companies within industries, based on nine different criteria.  To create the industry rankings, surveys were sent to over 3,800 executives, directors, and analysts to select the 10 companies they admired most.
